Implant-supportef over-dentures are used in not dentate patients, i.e. patients who have no teeth at all. They are ideal choice both for elderly patients and the younger who suffer insufficient retention of their full dentures. It takes 3-4 implants to restore a non-dentate upper jaw and 1-2 for the lower jaw. Implants are connected either with a metal bar, which provides stiffness and retention to the over-denture or with cylindrical locators. Over-dentures are treatment of choice for patients bearing highly absorbed mandibular bone, making it impossible to place more implants for supporting a fixed bridge. Such patients often suffer a poor denture stability and poor mastication. Placing only two implants in the lower jaw followed by adjustment of existing denture offers a surprising improvement in quality of life. Implant-retained dentures are less expensive than fixed prosthetic restorations and are to be considered a real game-changer, in cases that fixed restorations cannot be fabricated.
